Comments/Explanation:	There is no riser or maintenance hole on this very old & deep tank so it was pumped & back flushed through the inspection pipe. The owner signed the form for pumping through the inspection pipe. Performed a visual inspection & lowered a camera & light into the empty tank - no inlet baffle, heavy root infestation, signs of leakage & possible collapsing of the walls.
