    7/14,(SB) tank integrity, page 2 uploaded. 3c is still incomplete.. I spoke with Jeff over phone, he was driving and said the system was not non-protective of groundwater, said that was just an honest mistake and also doesn't know how the tank integrity and/or page 2 went missing as that was always there to begin with. I told him I would accept his report based on our verbal conversation and asked him to submit a new form or revised component #5 so that all boxes are verified. 
    6/30 called Jeff about tank integrity as homeowner wants the county acknowledgement of COC for building permit purposes. Need tank integrity, page 2 of report and 3c is incomplete
